Transitioning Youth 
Creating A Plan of Possibility

We assist young adults with intellectual and developmental disabilities (IDD) take their next step after high school. Our Transitioning Youth services are designed to support youth as they move from school into adult life — with a focus on employment, independence, and community engagement.
 

We work alongside youth, families, and support teams to create a future grounded in each person’s goals, strengths, and voice.

Who We Support​

We serve youth ages 18 and older who are eligible for services through the Maryland Developmental Disabilities Administration (DDA) and/or Department of Rehabilitative Services (DORS). We collaborate with families, Coordinators of Community Services (CCS), schools, and community organizations to ensure a seamless and successful transition into adulthood.

Every journey is unique.

 

Each plan is created in partnership with the person, their families, and team and reflects the strengths, goals, and interests, and builds a pathway to independence.
